Overview

This education explains the importance of Disease Burden Capture (DBC), which is the process of accurately coding each patient’s diagnoses so their full health status is reflected. Accurate DBC is required by CMS for Medicare and Medicare Advantage patients and influences a patient’s RAF score, which directly affects reimbursement. The module also covers how HCCs group related diagnoses for risk-adjusted payment models and how accurate documentation supports quality improvement efforts. By the end, learners will understand the financial impact of proper coding and how DBC aligns with problem list management and problem-oriented charting.

The basic concepts of Disease Burden Capture are available for review in another Workday Learning module entitled “CDI: Introduction to HCC Documentation and Queries.” This course intended to build on that foundation.


Objectives
At the conclusion of this activity, learners will be able to:

  1. Describe the synergies between correct disease burden capture coding and other quality improvement efforts.
  2. Detail the financial impact of accurate disease burden capture coding across an integrated healthcare delivery system.
  3. Illustrate how accurate DBC is the natural and efficient progression from Problem List Management and Problem Oriented Charting.
